Waiver & Release Form
All Summer Celebration participants must complete and submit an online waiver and release form no later than Wednesday, June 11. This form will be made available here one month prior to the event.
Upon submission of the form, an email will be sent to complete the process and to produce a copy of the signed agreement for download.
